TRANSPORTATION ENROLLMENT FORMS
This form is currently accepting responses. The form will be paused at 4:00 PM on Friday, July 25th to allow for routing for the 2025 -2026 School Year. The form will reopen on Wednesday, August 6 at 8:00 AM. If you require assistance with this form please contact our office at 831-645-1207.
Please note: Families will need to complete a new form for each school year. You can request transportation for summer and fall transportation services within the same form. If your child's address, emergency contact information, or school change during the school year a new form will be required.

ABOUT THE MPUSD SPECIAL EDUCATION DEPARTMENT
The MPUSD Special Education Department provides services to children with disabilities between the ages of birth through 22 years. Specialized Academic Instruction (SAI) is provided to address the educational and related needs of children with disabilities. All service needs are determined by IEP teams. The services across the district include:
- Infant/Toddler and Pre-School Special Education Program
- Resource Specialist Teachers at each of the District Schools
- Speech/Language Therapy
- Occupational Therapy
- Adapted Physical Education
- Assistive Technology
- Deaf and Hard of Hearing
- Self-contained classes with specialized academic instruction for students whose needs require mild to moderate adaptations to the general education curriculum that cannot be provided in a general education setting.
- Self-contained classes (preschool through adult transition) with specialized academic instruction for students who require curriculum based on life skills curriculum standards that cannot be provided in a general education setting.
- Self-contained classes (kindergarten through high school) with specialized academic instruction for students whose needs require a therapeutic setting with embedded mental health supports that cannot be provided in a general education setting.
- Self-contained classes (preschool through high school) with specialized academic instruction for students who are Autistic and whose needs require extensive language scaffolds and supports that cannot be provided in a general education setting.
Referrals for Assessment to Determine Special Education Eligibility
Referrals for special education evaluation for current MPUSD students can be submitted to your child's school. Please provide this referral in writing to the school psychologist on your child's campus.
Referrals for special education evaluation for children who attend a private school within the MPUSD attendance can be submitted to Heather Cheatham at [email protected].
Referrals for children who are not yet in kindergarten can be submitted directly to the MPUSD Birth to 5 Program: (Early Start)
